At Centauri Care we understand and appreciate that moving to a new home can be a traumatic and challenging experience for a young person and their families. We will ensure the placement referral is appropriate in relation to the home’s current residents. For each referral we will conduct:

• A review of the young person’s care plan, history and circumstances.
• Assess the needs of the young person against the homes Statement of Purpose.
